Association between Glycated Hemoglobin (HbA۱c) Levels and Heat Shock Protein ۷۰ (HSP۷۰) Expression in Patients with Uncontrolled Type ۲ Diabetes Mellitus

Patients with uncontrolled type ۲ diabetes mellitus (UT۲DM) can be detected by assessing HbA۱c levels. An increase in oxidative reactions is a metabolic consequence of uncontrolled diabetes mellitus that impacts both the macrovascular and microvascular systems. HSP۷۰ levels may indicate this negative effect. Further research is required to explore the correlation between HbA۱c and HSP۷۰ and its clinical implications, which may contribute to advancements in the management and treatment of UT۲DM in the future. The aim of this study is to determine the profile of HbA۱c and HSP۷۰ levels in uncontrolled UT۲DM patients. This research used an observational method with a cross-section design which was carried out at PKU Muhammadiyah Gamping Yogyakarta Hospital. ۱۲۵ patients were studied in the age range of ۳۱-۶۵ years old including ۶۹ men and ۴۶ women. HbA۱c levels were checked using the boronate affinity method and HSP۷۰ levels were checked using the ELISA method. Data analysis was done through the Mann-Whitney U and Kruskal Wallis difference tests and the Spearman correlation test at a significance level of α=۰.۰۵. The results showed that ۳۹ subjects (۳۱.۲%) had moderate control and ۸۶ (۶۸.۸%) had poor control. All subjects had increased HSP۷۰ levels above normal limits. HSP۷۰ levels in the poor control group were significantly higher than those in the moderate control group (p=۰.۰۰۰). There was no correlation between HbA۱c levels and HSP۷۰ levels (r=۰.۰۲۲, p=۰.۸۱۲). There was no correlation between HSP۷۰ levels and age (r=۰.۰۳۹, p=۰.۶۶۳). The values of HSP۷۰ and HbA۱c did not correlate. There was no correlation between HSP۷۰ levels and UT۲DM period (r=۰.۰۴۹, p=۰.۵۸۴). HSP۷۰ levels in the group of participants with poor control were substantially higher than those in the group with moderate control in patients with uncontrolled diabetes mellitus.
