Effect of Oral L-Carnitine and Omega-۳ on Lipid Profiles, Sleep Quality, and Quality of Life in Hemodialysis Patients

Aims: This study aimed to evaluate the possible beneficial effects of omega-۳ and L-carnitine, as well as their combination, on individuals undergoing hemodialysis. Materials & Methods: This randomized clinical trial was conducted on ۷۵ hemodialysis patients, divided into three groups (each ۲۵ participants) using a randomized block method. The intervention groups received L-carnitine ۲۵۰ mg tablets twice a day, one omega-۳ capsule, ۱۰۰۰ mg per day, and L-carnitine ۲۵۰ mg tablets twice a day along with one omega-۳ capsule, ۱۰۰۰ mg per day, for three months. Lipid profiles and C-reactive protein levels were measured through laboratory tests before and after the intervention. The quality of life for kidney disease and quality of sleep were assessed using the Kidney Disease Quality of Life Questionnaire and the Pittsburgh Sleep Quality Index, respectively. Data were analyzed using SPSS ۲۰ software. Findings: The administration of L-carnitine (۵۰۰ mg per day) did not demonstrate a beneficial effect on high-density lipoprotein, low-density lipoprotein, triglyceride, and C-reactive protein serum levels (p>۰.۰۵), but it improved the quality of sleep. Omega-۳ supplementation (۱۰۰۰ mg per day) decreased triglyceride and low-density lipoprotein levels, although these reductions were not significant. Additionally, omega-۳ treatment significantly improved the quality of life in kidney disease and the quality of sleep in patients undergoing hemodialysis (p<۰.۰۵). The coadministration of L-carnitine with omega-۳ did not show superior effects compared to omega-۳ alone in the evaluated parameters. Conclusion: Omega-۳ supplementation, both alone and in combination with L-carnitine, improves the quality of sleep and life in hemodialysis patients.
