The influence of HLA-DRB۱, -DQB۱ genes on age at onset of type ۱ diabetes in Iranian T۱D patients

Objective: Survey of the influence of HLA-DRB۱, -DQB۱ alleles, genotypes and haplotypes on age at onset of type ۱ diabetes (T۱D) in an Iranian population Materials and Methods: ۱۰۵ Iranian T۱D patients of different ethnic group and ۱۰۰ ethnically, age and sex matched individuals were selected from Tehran's hospitals and HLA-DRB, -DQB typing was performed. According to the age at onset of T۱D, the patients were divided into ۴ groups (۱-۵, ۶-۱۰, ۱۱-۱۵, ۱۶-۲۰ years). The frequency of susceptible and protective alleles, genotypes and haplotypes was calculated in each group. The data were evaluated by using fisher's exact test. Odds Ratio or relative Risk was measured for all samples. Results: The results illustrated that the frequency of the HLA-DRB۱*۰۴۰۱ allele decreased with increasing age, whereas the frequency of the HLA-DQB۱*۰۲۰۱ allele increased with increasing age. The HLA-DRB۱*۰۳۰۱ and HLA-DQB۱*۰۳۰۲ alleles demonstrated the highest frequency in the ۶-۱۱ and ۱-۵ years age at onset group, respectively. HLA-DRB۱*۰۴۰۱-DQB۱*۰۳۰۲ haplotype had the most frequency among the ۱-۵ years age at onset group (p: ۲×۱۰-۷, OR: ۶۹.۹۱۹) and the frequency of HLA-DRB۱*۰۳۰۱-DQB۱*۰۲۰۱ haplotype was the highest in the ۶-۱۱ years age at onset group among others (p: ۲×۱۰-۶, OR: ۶.۲۴۳). Conclusion: The current study indicated that HLA-DRB۱, -DQB۱ alleles, genotypes and haplotypes are associated with age at onset of type۱ Diabetes in Iranian T۱D patients. The individuals carrying alleles that are associated with younger age at onset should take care under preventive treatment.
