Diabetic cardiomyopathy is caused by the gradual weakening of the heart muscle due to prolonged high blood sugar levels. This condition affects ۶۰% of the ۳۸۰ million people with diabetes worldwide. Diabetic cardiomyopathy is usually asymptomatic in the early stages. One of the first signs is left ventricular hypertrophy, characterized by impaired filling of the ventricle during early diastole, increased filling of the atria, and prolonged volume resting phase. Left ventricular dilation and symptomatic heart failure occur after the onset of systolic dysfunction. In fact, recent research findings indicate that diastolic dysfunction is associated with impaired cardiac insulin signaling. Several mechanisms have been identified that contribute to reduced cardiac function in diabetes and have been investigated to date. These mechanisms include the exposure of the heart to a hyperglycemic diabetic environment along with increased fatty acids (FA) and cytokines. This article reviews the molecular mechanisms of diabetic cardiomyopathy. To better understand the exacerbating factors of diabetic cardiomyopathy and to explore therapeutic and control pathways, it is essential to investigate and test the signaling and molecular pathways mentioned.
