Investigation of Factors Affecting Postoperative Nausea and Vomiting in Pediatric Diabetic Patients Undergoing Orthopedic Surgeries

Background: Postoperative nausea and vomiting (PONV) is a common complication in pediatric patients, particularly those with diabetes undergoing orthopedic surgeries. This study aims to investigate the factors contributing to PONV in this vulnerable population. Methods: A prospective observational study was conducted with ۱۰۰ pediatric diabetic patients undergoing orthopedic surgeries. Data on demographics, duration and control of diabetes, type of anesthesia, opioid use, and antiemetic prophylaxis were collected. The incidence of PONV was assessed in the postoperative period. Results: The incidence of PONV was ۳۸%, with significant associations identified for female gender (p=۰.۰۲), poor glycemic control (p=۰.۰۱), longer duration of diabetes (p=۰.۰۳), and use of general anesthesia (p=۰.۰۴). Patients receiving intraoperative opioids had a higher incidence of PONV (p=۰.۰۱), while those receiving prophylactic antiemetics experienced significantly lower rates (p=۰.۰۳). Conclusion: PONV is prevalent in pediatric diabetic patients undergoing orthopedic surgeries, influenced by several modifiable and non-modifiable factors. Tailored perioperative strategies, including the use of regional anesthesia and antiemetic prophylaxis, may reduce PONV incidence in this population.
