Barriers to telephone CPR from the point of view of EMD in Iran, Kermanshah province: a qualitative study

چکیده مقاله:

Back ground: Telephone Cardio Pulmonary Resuscitation (T-CPR) by Emergency Medical Dispatchers (EMDs) to the bystanders who are near the out of hospital arrest’s patients without the condition of the patient being visible, is a very difficult task and in this way has many barriers and problem. There is lack of literature regarding T-CPR in Iran and this study explore the barriers to telephone CPR from the point of view of EMDs in Iran, Kermanshah province.Method: This research was conducted with a qualitative and descriptive phenomenology approach on ۱۲ EMD working in the dispatch of the Kermanshah emergency medical center, all of participants were female and with an average age of ۲۹ years. The data was collected through in-depth and semi-structured interviews with the participants selected using a purposive sampling method. The data were analyzed thematically using the seven-step approach of Colaizzi with MAXQDA۱۰ software.Finding: During the current study, the main theme include barriers to telephone CPR and sub-themes:.۱) tensions associated with patient transfer ۲) psychological tensions associated with T-CPR ۳) perceived tensions around bystander actions ۴) perceived need closed –circuit television (CCTV) for CPR and ۵) the tensions of the surrounding the bystander were extracted.Conclusion: The participants have stated that T-CPR in our country mostly fails due to the low level of public education, Cultivation and lack of awareness of the emergency function and the level of actions and training at the time of call People with ۱۱۵ are very difficult and often unsuccessful due to the mentioned barriers as well as the lack of visibility of the patient’s condition and the bystanders. On the other hand it is necessary that the EMDs in the field of effective communication with restless and confused bystanders and the way of training in a simple and understandable way as well as the management of stressful situations have high skills in this field. The quality of EMS due to the lack of required infrastructure and equipment which includes emergency centers inside or outside the city and equipped ambulances is under the negative effects of this problems.
