Background: Mental health disorders represent a significant portion of the global burden of disease, yet mental health services remain inadequately integrated into primary healthcare systems in many countries. This systematic review aims to examine the effectiveness and challenges of integrating mental health policies into primary healthcare, highlighting successful strategies and areas for improvement.Objectives: The primary objective of this systematic review is to assess the impact of mental health policy integration into primary healthcare systems. Specifically, the review aims to identify effective approaches, evaluate their outcomes, and explore barriers and facilitators to successful integration.Methods: A comprehensive literature search was conducted across multiple databases, including PubMed, Scopus, PsycINFO, and Web of Science, to identify studies published between January ۲۰۰۰ and December ۲۰۲۳. Inclusion criteria encompassed peer-reviewed articles, randomized controlled trials, cohort studies, and qualitative research that evaluated the integration of mental health policies into primary healthcare. Data extraction and quality assessment were performed independently by two reviewers, with discrepancies resolved through consensus.Results: The review identified ۷۰ studies meeting the inclusion criteria. Interventions were categorized into five main types: policy development and implementation, training of primary healthcare providers, integration of mental health services into primary care settings, community-based mental health programs, and use of digital health tools. Policy development and implementation were found to be critical for establishing a supportive framework and funding. Training programs for primary healthcare providers significantly improved the detection, diagnosis, and management of mental health disorders. Integration of services into primary care settings led to increased accessibility and reduced stigma. Community-based programs effectively engaged local populations and provided culturally appropriate care. Digital health tools, including telemedicine and mobile health applications, enhanced service delivery and patient engagement.Discussion: The findings underscore the importance of a multifaceted approach to integrating mental health policies into primary healthcare. Successful integration requires strong policy frameworks, adequate training for healthcare providers, and the adaptation of services to local contexts. Barriers to integration included limited resources, insufficient training, and stigma associated with mental health disorders. Facilitators included government commitment, community involvement, and the use of technology to bridge service gaps. The review highlights the need for ongoing support, funding, and research to sustain and scale up successful integration models.Conclusion: Integrating mental health policies into primary healthcare is essential for improving access to and quality of mental health services. Effective strategies involve comprehensive policy frameworks, provider training, community engagement, and leveraging digital health tools. Future efforts should focus on addressing barriers, fostering collaboration among stakeholders, and ensuring sustainable funding. Continued research is crucial to refine integration approaches and enhance mental health outcomes globally.
