Background: The first aim is to find whether rate of shorteningin cervical length is associated with preterm birth in twins.Materials and Methods: This is a prospective cohort of serialtransvaginal cervical length performed for twin pregnancies ata single institution from ۲۰۱۳ through ۲۰۱۶. Women with diamniotictwin pregnancies who had transvaginal cervical lengthmeasurements at ۱۸ and ۲۴ weeks' gestation and follow up resultswere observed. Statistical methods were used to assess theassociation between the rate of alter in cervical length and therisk of premature birth<۳۴ weeks.Results: Overall, ۲۶۴ women had criteria for enter this work.The middling rate of alteration in length of cervix for patientswith preterm birth<۳۴ weeks was ۱.۵ mm/wk vs. ۰.۹ mm/wkfor who delivered ≥ ۳۴ weeks. Women with fast alteration intransvaginal cervical length, ≥ ۲ mm/wk, had ۳ times higher thelikelihood of spontaneous preterm birth as those with slowerchange.Conclusion: Shortening in transvaginal cervical length in the۱۸ and ۲۴ w is related with spontaneous preterm birth, so guidelinesfor sequential transvaginal cervical length checking canhelp the clinician to discover at-risk patients. ≥۱.۵ mm/wk decreaseof cervical length identifies patients with prominent riskfor spontaneous preterm birth under ۳۴ weeks.
