Colorectal adenomas are commonly encountered lesions that have the potential for malignant transformation. Colorectal cancer is the third most common cancer worldwide. Over the period of ۷–۱۰ years, tumors that begin as benign adenomas can develop into malignant carcinomas. Adenomas can be further subdivided into tubular adenoma, tubulovillous adenoma, and villous adenoma, based on their histopathological features. Villous adenomas are the rarest subtype with the greatest tendency for malignant transformation. Villous adenomas are usually asymptomatic and are most commonly found in the rectosigmoid area. We present a case of a ۶۱- year-old male with ۳ positive fecal occult blood tests following colorectal screening. The patient was asymptomatic. He denied any symptoms such as changes in bowel habits, abdominal pain, unintentional weight loss, or visible rectal bleeding. He underwent a colonoscopy that revealed a ۵.۵ cm tumor in the transverse colon. Both the gastroenterologist and the surgeon believed that the tumor was malignant. Given the size and the cauliflower nature of the adenoma, the patient was referred for a right hemicolectomy. The post-surgical histopathological report concluded the presence of a ۵.۵ cm tubulovillous adenoma with low-grade dysplasia and two further polypoid lesions measuring ۱.۲ cm and ۱.۸ cm in the transverse colon. An abscess was formed following the surgery which required incision and drainage. The patient was completely asymptomatic, in spite of the large size of the tumor which almost obstructed the transverse colon. This case highlights the significance of early screening and timely intervention to prevent the progression of adenomas to carcinomas. Managing patients within a multidisciplinary team can further aid in providing high-quality patient care. Having regular colonoscopies is of paramount importance when it comes to the management of polyps, as surgical resection of benign polyps can be associated with higher rates of morbidity and mortality. This also indicates the need for improved interventions in the future. مقدمه/ بیان مساله : Colorectal cancer is the second most common cause of cancer-related mortality globally. Adenomas can undergo malignant transformation. Hence, their presence plays an important role in the underlying etiology of colorectal cancer. Adenomas can be categorized into tubular, tubulovillous, and villous adenomas based on their histopathological characteristics.
