Introduction: Giant-cell fibroma (GCF) is a localized, benign fibrous mucosal mass, that clinically is the same as any other fibroepithelial growth. However it is not associated with chronic irritation and can be distinguishes by its clinicopathologic features. It usually occures during the ۲nd and ۳rd decades of life and is an asymptomatic sessile or pedunculated nodule which usually is less than ۱cm in size. Approximately ۵۰% of all cases appear on the gingiva and there is a slight female tendency. The surface of this lesion often appears papillary; therefore can be in clinically mistaken for Papilloma, but definite diagnosis can only be made by histopathological examination.Case Report: A ۵۶-year-old female patient referred to the Department of Oral and Maxillofacial surgery, Alborz University of Medical Sciences, with a pinkish sessile mass with papillary surface on the right buccal mucosa near to commissue of lip. excisional biopsy of the lesion was done under general anesthesia and sent for histopathological examination. On gross examination the lesion consists of a piece of irregular creamy tissue measuring ۰.۳ x ۰.۲ x ۰.۱ cm. Microscopic examination illustrates ortho to parakeratinized squamous epithelium with elongated rete ridges (Figure ۱). The underlying connective tissue is fibrotic and contains large stellate fibroblasts within the superficial part (Figure ۲ and ۳). Based on histopathologic features, giant cell fibroma was the final diagnosis. Treatment for these lesions is conservative surgical excisions and the recurrence is rare. The different diagnosis stated for this lesion were Squamous papilloma, Verruca vulgaris, and Irritation fibroma. Conclusion and Discussion: Giant Cell Fibroma unlike other fibrous overgrowths, is a harmless lesion. The stroma displays the characteristic presence of giant fibroblasts which are stellate or spindle-shaped, with ۲ or ۳ nuclei. The GCF is an unusual entity among elderlies and is most prevalent in first three decades of life. GCF has the inclination to grow on the gingiva. Conservative surgical excision is the decisive treatment for this lesion.
