Background and aim: The purpose of this study was to investigate the frequency of renal involvement and its characteristics in patients with Henoch-Schonlein purpura syndrome referred to Hazrat Masoumeh Hospital in Qom.
Materials and methods: In this retrospective study, the files of ۴۹ patients with Henoch-Schonlein purpura disease were reviewed, and all clinical and paraclinical findings of the patients were reviewed, and the prevalence of renal complications in these patients was evaluated.
Results: In this study, ۲۸ boys (۵۷.۱%) and ۲۱ girls (۴۲.۹%) were examined. The average age of patients in this study was ۵۳.۷ ± ۳۵.۳ months. ۳ patients (۶.۱%) had underlying disease, ۳ patients (۶.۱%) had edema, ۱۸ patients (۳۶.۷%) had arthritis, ۹ patients (۱۸.۴%) had petechiae, ۱۰ patients (۲۰.۴%) had abdominal pain. ۷ patients (۱۴.۳%) had leukocyturia, ۹ patients (۱۸.۴%) had micro hematuria and hematuria, ۳ patients (۶.۱%) had proteinuria, and the average creatinine was ۰.۶ ± ۰.۱ and the average blood urea of these patients was ۲۵.۴ ± ۱۱.۱. Creatinine was increased in ۵ cases (۱۰.۲%) at the beginning of diagnosis and in ۳ cases (۶.۱%) in the second week. ۸ patients (۱۶.۳%) had kidney failure and in three of these patients we saw an increase in proteinuria at the beginning of the diagnosis, also in three patients there was recovery, two of these three patients two weeks later and one patient In the next ۶ months there was an improvement. In this study, we had disease recurrence in ۱ patient (۲%), nephrotic syndrome in ۱ patient (۲%), and proteinuria in ۲ patients. In ۳۳ cases (۶۷.۳%) no treatment was needed, in ۱۰ cases with NSAID and in ۳ cases (۶.۱%) with Lasix and the same number of cases were treated with corticosteroids. Also, among the patients who received treatment, ۱ patient (۳%) did not respond to the treatment
Conclusion: Considering the kidney complications of this disease and the need for long-term follow-up for timely diagnosis, if any child is diagnosed with similar conditions and follow-up of patients, it can be controlled and prevented to a large extent.
