Clinical Manifestation of Acute Myocardial Infarction: Classified by Age and Gender

Background: The purpose of the study was to evaluate clinical manifestations of acute myocardial infarction as classified by age andgender. Methods: This cross sectional study was conducted in a coronary care unit of a regional urban medical center in northern Iran (Amol). Atotalof ۳۶۶patientsdiagnosedwithacutemyocardialinfarctionwererecruitedfromJanuarytoJune۲۰۱۳. Patientdemographicinformation,pastmedicalhistory,andcurrentsymptomdatawerecollected. Results:Multivariatelogisticanalyseswereperformedtoidentifyriskpredictorsofmyocardialinfarctionpatientsclassifiedbyage andgender.Riskpredictorsforolderpatientsweredyspnea,OR=۱.۷۶(۹۵% CI۱.۰۱,۳.۰۶),weaknessoddsratio=۲.۳۵(۹۵% CI۱.۳۱,۴.۲۱), nausea odds ratio = ۱.۸۳ (۹۵% CI ۱.۰۴, ۳.۲۰), vomiting odds ratio = ۲.۴۸ (۹۵% CI ۱.۳۴, ۴.۵۷), fatigue odds ratio=۱.۸۷ (۹۵% CI ۱.۰۲, ۳.۳۹), belching, odds ratio = ۲.۱۳ (۹۵% CI ۱.۰۸, ۴.۲۰), and hiccups odds ratio = ۲.۸۱ (۹۵% CI ۱.۲۵, ۶.۳۰). Sub group analysis in older women patientsidentifiedweaknessoddsratio=۳.۱۳(۹۵% CI۱.۱۱,۸.۸۵),andbelchingoddsratio=۳۴.۷۰(۹۵% CI۳.۸۶,۳۱۲.۲)asriskpredictors. Amongoldermenpatients,thepredominantsymptomsweresweatingoddsratio=۳.۷۴(۹۵% CI۱.۰۶,۱۳.۲)andvomitingoddsratio =۲.۵۴(۹۵% CI۱.۱۰,۵.۹۱). Conclusions:Thisstudyconcludesthatolderacutemyocardialinfractionpatientsweremorelikelytohavenon-specificsymptoms. Initial assessment for acute myocardial infarction should consider the possibility of non-specific clinical manifestations, such as weaknessandbelchinginolderwomen,andsweatingandvomitinginoldermen.
