Clinical Manifestations and Associated Mortality Factors of COVID-۱۹: A Large Population-Based Study in the Northeast of Iran During ۲۰۲۰-۲۰۲۱

Background and aims: The emergence of the coronavirus disease ۲۰۱۹ (COVID-۱۹) has become oneof the greatest health problems of the ۲۱st century. The current study was conducted to investigateCOVID-۱۹’s clinical manifestation, mortality factors, and their association with each other during thethree devastating waves of the pandemic in Razavi-Khorasan province, Iran.Methods: This cross-sectional epidemiological population-based study was performed in Razavi-Khorasan province, Iran from January ۲۱, ۲۰۲۰, to March ۲۰, ۲۰۲۱. The data, including demographiccharacteristics and clinical presentations of the patients, were extracted from the Medical CareMonitoring System (MCMS), disease management portal in the Deputy of Health and hospitalinformation system (HIS) of the medical universities/faculties of the province.Results: Overall, ۸۰ ۴۹۹ patients were admitted to all hospitals of Khorasan-Razavi with the laboratory/clinical COVID-۱۹ confirmed disease. The male-to-female ratio and the mean age of our COVID-۱۹individuals were ۱.۱۰:۱ and ۵۵.۶۷ ± ۲۳.۲۷, respectively. The most frequently reported presentingsymptoms in histories provided at the admission time were respiratory distress (۵۸.۲%), fever (۳۶.۷%),and cough (۳۴.۹%), while the less common ones were abdominal pain (۲.۷%) and alternation in smell/taste (۰.۹%). Male gender (odds ratio [OR] = ۱.۳۲, P < ۰.۰۰۱), age over ۶۰ (OR = ۲.۵۹, P < ۰.۰۰۱), andpresence of at least one comorbidity (OR = ۱.۳۲, P < ۰.۰۰۱) were significantly associated with highermortality rates.Conclusion: Healthcare providers and public health managers can benefit from the findings of thisstudy to detect and emphasize patients with poor prognoses.
