چکیده مقاله:

Objectives:Concerning the high Covid-۱۹ prevalence and mortality and the overridingimportance of using non-invasive ventilation (NIV), the disease treatment is presently a leadingchallenge, especially among ICU-admitted patients. This way, the present study was conductedto determine the Effective factors on CPAP Failure Rate in Covid-۱۹ related hypoxemia.Methods:This is a retrospective cross-sectional study (۲۰۲۱) was conducted on ۲۰۰ Iranianadult Covid-۱۹ patients with acute respiratory failure (ARF) admitted to the ICU (Pao۲/fio۲ <۳۰۰ mmHg; PaCO۲ < ۴۵ mmHg) who undergo continuous positive airway pressure (CPAP)therapy.Measurements:The HACOR scale (heart rate, acidosis, consciousness, oxygenation, andrespiratory rate) was measured before and an hour after undergoing the CPAP treatment.Patients' demographic, anthropometric, and laboratory data were recorded and analyzed inSPSS-۲۲ software using the Mann-Whitney, Chi-square, Wilcoxon, and logistic regressiontests at p≤۰.۰۵.Results:The mean age of patients was ۶۳.۹۶ ± ۱۶.۲۳ years. Of all ۲۰۰ patients ۷۸.۵%experienced CPAP failure, and the remaining ۲۱.۵% underwent successful CPAP therapy.Diabetes mellitus (DM) and HACOR score an hour after CPAP controlled CPAP failure amongpatients. CPAP failure chance among DM patients was ۱۴.۹۲% higher than in non-DMpatients. It was also ۷.۱۰% higher in patients with higher HACOR scores an hour after CPAPthan in others. Additionally, obesity and elevated blood urea nitrogen (BUN) impacted CPAPfailure rates among patients.Conclusion:HACOR score an hour after CPAP, DM, old age, obesity, and elevated BUN favorelevated CPAP failure rates among patients. The mortality rate is much higher in patients withCPAP failure.
