Background: Uterine transplantation (UTx) is a complex procedureinvolving extensive abdominal surgery on the donor andrecipient. This procedure has not yet been recognized as thes tandard treatment for absolute uterine factor infertility (AUFI).The present s tudy aimed to determine the risks of UTx.Materials and Methods: In this review s tudy, the PubMeddatabase was searched, with no time, language, or location restriction until March ۱, ۲۰۲۲, using the words "uterine transplantation”or "uterus transplantation”. Only articles describingthe risks of UTx in real procedures performed on humans wereincluded in the s tudy.Results: From ۷۲۰ identified articles, ۴۰ papers were enteredthe s tudy for qualitative review. Based on the results, removalof the uterus from a living donor is a very invasive surgery.Surgical and pos toperative risks, including bleeding, infection,ureteral injury, and psychological risks such as depression,anxiety, and decreased quality of life can occur for the donor.The recipient is also exposed to the risks associated with invitro fertilization (IVF), the risks associated with transplantationsurgery, cesarean section, hys terectomy and psychologicalrisks. Exposure to immunosuppressive drugs and a high riskof developing complications during pregnancy such as pretermdelivery, preeclampsia and ges tational diabetes are among thefactors that threaten the health of the fetus and neonate.Conclusion: UTx is associated with risks for the living donor,recipient, fetus and neonate. Increasing the number of UTx proceduresincreases our unders tanding of its risks. The developmentof less invasive methods, such as robotic surgery, reducesthe complications of surgery in a living donor
