Background: Polycys tic ovary syndrome (PCOS) is a verycommon endocrine disorder and one of the causes of infertilityamong women. Among the factors involved in the developmentof PCOS and its related symptoms, Oxygen reactive species(ROS) can be mentioned, which increases the number of atreticfollicles, which occurs as a result of an increase in apoptosis ingranulosa cells of the follicles. In this s tudy, NAC was used asa potent antioxidant to reduce oxidative s tress and thus reduceapoptosis in granulosa cells in women with PCOS and its effecton apoptotic expression was s tudied.Materials and Methods: ۶۰ women entered the s tudy with specificInclusion and exclusion criteria and divided into ۳ groupsof ۲۰ people. The firs t group consis ted of ۲۰ women with PCOSwho received NAC ۶۰۰ mg daily for three consecutive days forsix weeks, The second group included ۲۰ women with PCOSwho received placebo three times a day for six weeks and Thethird group included ۲۰ women with normal ovarian functionand infertility problems due to male or mechanical factors. Follicularfluid was collected after oocyte pancreas in these patientsand granulosa cells were isolated. The expression of thepro-apoptosis genes such as caspase-۳ and Bax, and the antiapoptosisgenes such as Bcl-۲ and XIAP were inves tigated indifferent groups using the Real Time PCR. TUNEL assay wasalso performed to evaluate apoptosis rate in different groups.Results: Our results revealed a significant upregulation ofBCL۲ and XIAP and also downregulation of BAX and Caspase۳in NAC treated patients when compared with placebogroup. TUNEL assay showed NAC can decrease the apoptosisin PCOS women.Conclusion: Based on present s tudy prescription of NAC canimproved fertility rate and can be used for PCOS women
