Risk Factors of COVID-۱۹ Mortality in Northwest of Iran

Background: Coronavirus disease ۲۰۱۹ (COVID-۱۹) is a virulent disease caused by a novel coronavirus contaminating over ۲۰۰ countries. Data about the risk factors of the disease in Iran are limited and vary in different reports. We tried to assess the factors that increase the mortality risk in patients afflicted by COVID-۱۹. Methods: Confirmed COVID-۱۹ cases who were admitted in Tabriz from October to December ۲۰۲۰ were registered and followed up until their discharge or death. Applying multivariable binary logistic regression, possible factors that lead to mortality from COVID-۱۹ infection were analyzed. Results: From a number of ۲۶۰ cases that were focused on in the study, we identified ۱۶۵ patients with severe and very severe diseases. The average age of patients was ۶۲.۰۷±۱۷.۹۷ with ۷۴.۲% older than ۵۰ years. In the course of hospitalization, ۱۲۹ patients died. Factors linked with a higher risk of death in the hospital were among male patients (OR=۱.۸۶۱, P=۰.۰۱۶), very severe disease on admission (OR=۱.۰۳۴, P=۰.۰۳۴), no fever (OR=۲.۴۶۰, P=۰.۰۰۳), and hypotension (OR=۳.۹۰۰, P<۰.۰۰۱). Smoking acted as a factor reducing the risk of death in patients infected with COVID-۱۹ (OR=۲.۳۴۳, P=۰.۰۳۵). Conclusion: Male and elderly patients with an increased degree of PT, LDH, and ESR during their admission period were likely to die from COVID-۱۹ infection. The dominant risk factors linked to mortality were male gender, hypotension, and coagulation problems on admission. Also, mortality risk was lower in smoker patients compared with non-smokers.
