Background and objectives: Breast cancer is the most common malignant disease diagnosed in women worldwide. Screening for breast cancer is used to detect cancers at an early stage when they can be treated most effectively. Different types of the techniques applied in the early diagnosis and the screening of the breast cancer, such as ultrasound (sound waves) and mammography (X-ray screening). The aim of this study is to determine which is more accurate imaging test mammography or ultrasound for diagnosis of breast cancer based on the women’s age and breast density.Methods: We conducted grey literature searches using the following sources: OpenGrey; National Institute of Health Reporter; Health Services Research Projects in Progress.Results: According to the results of the study, sensitivity varied significantly with age and breast density. Previous studies show that, ultrasound had a higher sensitivity than mammography in women younger than ۴۵ years, whereas mammography had a higher sensitivity than ultrasound in women older than ۶۰ years. The sensitivity according to age for mammography and ultrasound was ۵۲,۱% and ۷۲,۶% Respectively. The specificity according to age for ultrasound and mammography was ۸۸, ۵% and ۷۳, ۹% Respectively. Comparing the sensitivity of mammography and ultrasound according to the breast density indicates that mammographic sensitivity was ۸۲,۲% among women with predominantly fatty breast, but ۲۳.۷% in women with heterogeneous dense breasts, with the increase of fibro glandular density the level of sensitivity with mammography decreases, while ultrasonographic sensitivity was ۷۱,۱% among women with predominantly fatty breast and ۵۷,۰% for heterogeneous dense breasts.Conclusion: this study indicate that breast density and age are important predictors of the accuracy of mammography. Breast ultrasound is more accurate than mammography in symptomatic women ۴۵ years or younger, mammography has progressive improvement in sensitivity in women ۶۰ years or older.
