Emerging of infections caused by multi-drug resistant (MDR)and Extensively-drug resistant (XDR) Acinetobacter baumannii inhospitalized patients in Iran; A systematic review.

Background and Aim : Acinetobacter baumannii is an opportunistic pathogen that has animportant role in causing hospital-acquired infections and increasing the rate of mortality amonghospitalized patients. Its resistance against many antibiotics has been significantly reported, thatindicates the complexity of treatment and also the prevention strategies of hospital infectionscaused by A. baumannii. This study aims to investigate the prevalence of multi-drug andextensively-drug resistant strains of A. baumannii in the last ۵ years.Methods : The international and Iranian national databases were explored according to the givenkeywords by Medical Subject Headings (MeSH). The search results were limited to the last fiveyears, from ۲۰۱۸ to ۲۰۲۲ in Iran. Twenty-four English and Persian original articles were selectedfor this study. The original papers containing clinical MDR (Multi-drug resistant) and XDR(Extensively-drug resistant) A. baumannii information were regarded as included criteria. Most ofthe studies have been done in Tehran.Results : A total of ۲۱۰۶ clinical isolates of drug-resistant A. baumannii were investigated in thosearticles. The results analysis showed that ۱۷۳۵ (۸۲.۳۸%) isolates and ۱۱۵۳ (۵۴.۷۴%) isolates wereconsidered as MDR and XDR, respectively. The selected studies were performed on various typesof clinical specimens, mostly wounds (including burn wounds) and respiratory tract samples(including tracheal aspirates). These specimens were taken from different wards of hospitals,mainly ICUs and burn wards. Antibiotic susceptibility patterns indicated that most of A. baumanniiisolates (۷۸.۱%) were resistant to ceftazidime and ۲۰.۹% of isolates were sensitive to PolymyxinB.Conclusion : This review shows the high prevalence of drug-resistant A. baumannii circulating indifferent wards in many Iranian hospitals recently. The necessity of rapid and accurate diagnosisof the infections caused by MDR and XDR strains, also finding novel effective antibiotics mustbe considered as effective rules to reduce the high mortality among hospitalized patients.
