Clinical manifestations and paraclinical findings of brucellosispatients admitted to Imam Reza Hospital, Mashhad – a ۱۰-yearretrospective study

Background and Aim : Brucellosis remains a common infectious disease in Iran, with highlyvariable clinical manifestations leading to its inclusion in the differential diagnosis of manydiseases. This study aims to investigate the common patterns of presentation, and the outcome ofpatients diagnosed with brucellosis in Imam Reza Hospital of Mashhad, Iran.Methods : In this retrospective cross-sectional study, we reviewed the medical records of allpatients admitted to Imam Reza Hospital, Mashhad from March ۲۰۰۸ to ۲۰۱۷, who met clinicaland paraclinical criteria required for a diagnosis of brucellosis. Extracted data includeddemographic information, relevant personal history (occupational or dietary exposure, etc.),clinical manifestations, paraclinical findings, and outcome.Results : A total of ۲۰۹ adults (mean age ۴۵.۰۴±۱۹.۵۸), of whom ۱۱۷ (۵۶%) were male, and ۵۶children (mean age ۶.۸۵±۳.۷۲) were included. ۱۳۱ (۶۲%) lived in rural areas, where ۵۴ (۲۵.۸%)adults were occupied with animal husbandry. ۴۷ (۲۲.۵%) reported a history of previous afflictionwith brucellosis. A history of fever was reported in ۱۸۴ (۸۰.۴%), arthralgia in ۱۰۱ (۴۸.۳%), andweight loss in ۸۰ (۳۸.۳%) cases. The most common syndromes at presentations were arthritis (۸۴- ۴۰.۲%) and spondylitis (۴۲ – ۲۰.۱%). ۵۵ (۲۶.۳%) patients had Wright agglutination tests with atiter of ۱/۳۲۰, accounting for the most common measurement. Only ۱ patient died in the hospital,due to complication with endocarditis.Conclusion : The most common syndrome at presentation was arthritis, while fever and arthralgiawere the most common manifestations. The only mortality was due to complication withendocarditis.
