Critical COVID ۱۹ in Solid Organ Transplatation

چکیده مقاله:

Background and Aim : Considering the importance of assessing solid organ transplant, infectedindividuals with coronavirus disease ۲۰۱۹ (COVID-۱۹), and the lack of information in this regard,this descriptive study aimed to investigate the clinical features, immunosuppressive agents, andoutcomes of liver transplant patients in the critical phase of infection with COVID-۱۹.Methods : This descriptive cross-sectional study was conducted on ۱۲ critically ill liver transplantrecipients referred to Imam Reza and Montaseriyeh hospitals affiliated to Mashhad University ofMedical silences, Mashhad, Iran, within ۲۰۲۰-۲۱. This study was extracted from a thesis to obtaina specialist MD degree in infectious diseases (Code: ۹۸۱۸۱۸). The required data, includingdemographic and clinical information, were gathered and recorded in a checklist, and thecorrelations between variables were assessed in SPSS software (version ۲۴).Results : Hypertension, diabetes, and chronic kidney disease were reported in ۸۳.۳% (n=۱۰),۵۸.۳% (n=۷), and ۴۱.۶% (n=۵) of patients, respectively. The administration of Mycophenolic acidwas correlated with conjunctivitis (r=-۰.۶۷; P=۰.۰۲), weakness (r=۰.۷۷; P=۰.۰۰۶), and sore throat(r=-۰.۶۷; P=۰.۰۲). Ground glass opacity was reported in all patients, which was along withconsolidation in ۹۰.۹% of the cases, and acute pulmonary embolism was found in ۳۶.۳% of thesubjects. Finally, ۶۶.۷% (n=۸) of patients passes away. Among immunosuppressive agents, onlythe use of Mycophenolic acid was correlated with outcome (r=-۰.۷۷; P=۰.۰۰۶).Conclusion : Due to the high rate of mortality among liver transplant recipients in the criticalphase of COVID-۱۹, earlier and more aggressive treatment with antiviral and antibacterial agentsshould be performed in this group of patients.
