Background and Aim : The antibiotic susceptibility pattern and carbapenem resistancemechanisms of Enterobacteriaceae isolates from surgical site infections (SSIs) were investigatedMethods : Fifty-two carbapenem resistance Enterobacteriaceae isolated was isolated from SSIs.The disk diffusion and agar dilution methods were applied for determine the antibioticssusceptibility patterns. AmpC and efflux pump overexpression and carbapenemase mediatedresistance were detected by phenotypic methods. The genes carbapenemase encoding genes weredetected by the PCRResults : According to the minimum inhibitory concentration (MIC) and disk diffusion, a highlevel of resistance were observed to most class of drugs except colistin and amikacin. No effluxpump overexpression was observed in resistance to carbapenems. Among ۵۲ carbapenem resistantisolates, Klebsiella pneumoniae was most common (۴۱/۵۲; ۷۸.۸۴%%) followed by Escherichiacoli (۸/۵۲: ۱۵.۳۸%), and Enterobacter spp (۲/۵۲; ۳.۸۴%). AmpC overproduction was found inEnterobacter spp. According to the phenotypic test and PCR results, the resistance to carbapenemin E. coli and K. pneumoniae was observed to be associated with carbapenems production. Themost common carbapenemase gene was blaOXA-۴۸-like (۵۳.۸%) followed by blaKPC (۳۲.۶ %)blaNDM (۱۱. ۵۳ %), and blaVIM (۱.۲%)Conclusion : According to our results the incidence of carbapenems resistant Enterobacteriaceaeis at a worrying level. The most frequent mechanism of carbapenems resistance wascarbapenemase. We suggest reevaluation in the controlling program of SSIs causedEnterobacteriaceae in our health care centers
