Global prevalence and distribution of antibiotic resistance ofStenotrophomonas maltophilia clinical isolates: a systematic review andmeta-analysis

Background and Aim : Stenotrophomonas maltophilia (S. maltophilia) is an opportunisticpathogen in patients taking immunosuppressive therapy, mechanical ventilation, catheters, andthose hospitalized for long periods. Treatment of (S. maltophilia) is difficult because exhibitsextensive resistance to a variety of antibiotics and chemotherapeutic agents. The current studyaims to provide a systematic review and meta-analysis of distribution of antibiotic resistanceprofile among clinical isolates of S. maltophilia through case reports/case series and prevalencestudies.Methods : A systematic literature search was performed for original research articles published inMedline, Web of Science, and Embase databases from ۲۰۰۰ to ۲۰۲۱ Statistical analysis wasperformed using STATA ۱۴ software to report antibiotic resistance of clinical S. maltophiliaisolates worldwide.Results : ۲۲۴ studies (۳۹ articles were case reports/case series, and ۱۸۵ articles were prevalencestudies) were included in current study. A meta-analysis of prevalence studies demonstrated thatthe most antibiotic resistance all over the world was due to levofloxacin, TMP/SMX, andMinocycline (۰.۱۴۴, ۰.۰۹۲%, and ۰.۰۱۴ respectively). Evaluation of the results of the casereports/case series showed that, out of ۵۷ patients with antibiotic resistance, TMP/SMX (۳۶.۸۴%),Levofloxacin (۱۹.۲۹%), and Minocycline (۱.۷۵%) were among the most prevalent antibioticresistance in patients. The most resistance to TMP/SMX is reported in Asia (۱۹.۲۹%), Europe(۱۰.۵۲%), and America (۷.۰۱%), respectively.Conclusion : Considering that relatively high resistance to TMP/SMX has been recorded, moreattention should be paid to the drug regimen of patients in order to prevent the emergence of multidrugresistant S. maltophilia isolates.
