Evaluation and Identification of Carbapenem Resistant Genesin Klebsiella pneumonia Isolated from Hospitalized Patients in Ilam City

Background and Aim : Klebsiella pneumoniae is an important multidrug-resistant (MDR)pathogen, which is the most common cause of hospital and community-acquired infections. Thispathogen causes various infectious diseases, including urinary tract infections, bacteremia,pneumonia, and liver abscesses. The acquisition of antibiotic resistance genes and the possibilityof transferring resistance between pathogen strains leads to antibiotic resistance amongopportunistic pathogens. The aim of this study was to evaluate and identify carbapenem resistantgenes in Klebsiella pneumonia isolated from hospitalized patients in Ilam city.Methods : The study included ۴۵ Klebsiella pneumoniae isolates that were isolated from variousclinical specimens. Identification of these isolates was done using microbiological tests such asgram staining and differential media. The antibiotic resistance pattern of the strains wasdetermined by disk diffusion method according to CLSI ۲۰۲۱ standard. In this study, antibioticdisks gentamicin, cefotaxime, ciprofloxacin, amikacin, imipenem, ertapenem, meropenem anddoripenem were used. The frequency of blaIMI and blaGES genes in the isolates was determinedusing PCR method.Results : Out of a total of ۱۲۴ culture samples collected during one year from the hospitallaboratory, ۴۵ samples were identified as Klebsiella pneumoniae. The results of the antibioticsensitivity test showed that the most resistance is related to the antibiotic cefotaxime (۳۷.۸percent). Also, the antibiotic ertapenem (۸.۹%) had the lowest resistance rate. PCR results indicatethat blaIMI and blaGES genes are not present in any of the strains.Conclusion : Identifying antibiotic resistance in hospital infectious agents is a major challenge inthe treatment of infections. Based on the results, carbapenems are considered suitable options forthe treatment of Klebsiella pneumoniae infection. However, the occurrence of this level ofresistance is also worrying and indicates the need to pay attention to the proper prescription ofthese antibiotics.
