Background and Aim : Type ۱ diabetes (T۱D) is an autoimmune disorder, characterized by theproduction of autoantibodies to the pancreatic beta-cells. In previous studies, we indicated thatMycobacterium avium subspecies paratuberculosis (MAP) could be a potential risk factor for T۱D,suggesting that MAP infection could induce immune imbalance. Emerging evidence has alsoindicated that Human endogenous retrovirus (HERVs) may play a role in etiopathogenesis of TD۱.Indeed, the envelope proteins of HERV-W and HERV-K families have been detected in serum andpancreas of patients with T۱D, for these reasons we investigated the role of MAP and HERVs ina Sardinian pediatric population with T۱D by analyzing the humoral response and variation overtime toward peptides derived from both, the external pathogen and the endogenous retrovirus inT۱D patients (from the onset up to ۱۲ years from the diagnosis) and healthy controls.Methods : Statistical significative different response was observed in antibody titre against allMAP and HERVs peptides between patients at T۱D Onset vs. HCs (p<۰,۰۰۰۱). A statisticallysignificative and progressive decline was observed in antibody level as early as the first year ofthe disease. On the other hand, this pattern occurred with all peptides but not for HERV-K whichremains constant from the onset until the age of six years.Results : Our results support the hypothesis that both pathogens are linked to T۱D etiology, andalso that HERVs activation can be induced by certain infectious agent such as MAP. The resultsare in line with this hypothesis, given the high reactivity expressed in patients T۱D onset for allpeptide fragments evaluated. The progressive decrease in antibody titer over the years couldreflects the development of an active autoimmune process at the onset. A contribution of MAP inthis process could result from molecular mimicry with homologous epitopes such as Zinctransporter ۸ (ZnT۸) and Proinsulin (PI), leading to autoimmune responses.Conclusion : Taken together, these findings support the hypothesis that MAP and HERV couldact as risk factors for T۱D, suggesting that they may serve as potential biomarkers.
