Relationship between hypocapnia and febrile convulsion

Introduction: Febrile seizures are common during infancy and early childhood, particularly between ۳ months and ۵ years of age. Despite of different animal studies, it isnot known whether hyperthermia induced hyperventilation with subsequent hypocapnia and alkalosis is relevant to febrile seizures in human children. This study was designed to find out any association of febrile seizures with fever-induced hypocapnia. Method: This is a cross sectional study which was performed within ۲۰۱۶-۲۰۱۹ in Akbarhospital, a tertiary referral center of pediatrics in Mashhad, Iran. Axillary temperature was measured and venous blood gas analysis was done soon after admission and within ۲۴ hour of seizure onset. Data was analyzed using SPSS software version ۱۹.۰ software. Results: One-hundred fifty five patients were enrolled in the study. The mean age ofthem was ۱۶.۴۳± ۱۲.۰۲ months. The mean PCO۲ was much lower than normal range (۲۹.۳۲± ۳.۴۰ mmHg) and ۹۳.۴% of patients had hypocapnia (PCO۲<۳۵mmHg). Themean pH was ۷.۳۵± ۰.۱۱ and ۲۸.۵% of them had alkalosis (pH>۷.۴۵). The mean O۲ saturation was ۷۶.۸۸± ۱۳.۵۶ percent. Conclusions: Our findings demonstrated that febrile convulsion might be related to fever induced hyperventilation and causing hypocapnia is one of the precipitating factor in inducing seizures. Further cohort studies in large-scale population is needed.
