Outcomes of Temporary Hemi-Epiphysiodesis Using a New Device for The Treatment of Pediatric Valgus Knee Deformity: A Preliminary Report

Background: This study aimed to evaluate the efficacy of hemi-epiphysiodesis using a new device (X-plate) for thecorrection of genu valgum.Methods: In total, ۲۲ children with a total of ۳۴ pathologic knee valgus deformities underwent the procedure andwere followed up ۲۵.۳ months on average.Results: The mean time to clinical correction of genu valgum was ۱۰.۹±۲.۲ months. Furthermore, the meanpreoperative anatomic Lateral Distal Femoral Angle (aLDFA) and mechanical Tibia Femoral angle (mTFA) wereobtained at ۷۵.۱±۳.۸ and ۶±۲.۸ degrees, respectively. When clinical correction of the deformity was achieved, themean of aLDFA and mTFA corrections were determined at ۸.۹±۵.۳ and ۶.۵±۳.۲ degrees, respectively. The meanspeed of aLDFA and mTFA corrections were estimated at ۰.۸±۰.۴۵ and ۰.۶±۰.۳ degrees per month, respectively.The postoperative aLDFA and mTFA were measured at ۸۴±۴.۲ and -۰.۸±۲.۹ on average. With a mean of ۲۵.۳±۱۴.۵months at final follow-up, there were ۶ (۱۷.۶%) mild valgus knees, ۲۶ (۷۵.۵%) normal alignment knees, and only ۲(۵.۹%) mild varus knee within an acceptable clinical limit.Conclusion: Hemi-epiphysiodesis using X-plate at the distal physis of the femur is an effective and safe method forthe treatment of valgus knee deformity in children.Level of evidence: IV
