Evaluation of GAP Trauma Scores to Predict Prognosis of Multiple-trauma Patients in ۲۰۱۸-۲۰۱۹ in Yazd

Background: Early diagnosis of major trauma and rapid transmission of patients to appropriate therapeutic centers have always been issues of concern. Several prognostic models for rapid clinical decision-making and estimating the mortality rate of multiple trauma patients have been presented previously. The current study aimed to evaluate the GCS, Age, and systolic blood pressure (GAP) scores of patients with multiple trauma and determine the cut-off points of these scores for predicting mortality rates.Method: In this study, we used the calculated scores to predict the prognosis of trauma patients. Scores ranged from ۳ to ۲۴. Based on previous studies, the total scores were divided into ۳ risk groups: ۱. Low-risk group (score of ۱۹-۲۴ for GAP); mortality rate: %۱.۸. Moderate-risk group (score of ۱۱ - ۱۸ for GAP); mortality rate: %۲۱.۴. ۳. High-risk group (score ۳-۱۰for GAP); mortality rate: %۷۴.۲. This cross-sectional descriptive study was included ۱۷۰۵۳ patients with multiple trauma. Data regarding age, systolic blood pressure, and Glasgow coma score were collected. GAP scores were calculated.Results: In this study, we assessed ۱۷۰۵۳ patients with multiple traumas, of which ۱۳۶۶۷ were male and ۳۳۸۶ female. By GAP scores of these, ۱۴۷۴۶ people include low-risk trauma(%۹۸/۲), ۲۷۴ people include Moderate-risk trauma(%۱.۶۲), ۳۳ people include High-risk trauma(%۰.۱۹۳). According to hospital statistics ۷۳ trauma patients transferred with EMS died in the Yazd hospitals. By GAP scores of these, ۲۲ people include low-risk trauma (%۰.۱۳), ۲۶ people include Moderate-risk trauma(%۹.۴۸), ۲۵ people include High-risk trauma(%۷۵.۷۵).Conclusions: The death toll in low-risk group and moderate-risk group is lower than expected, while High-risk group is more than expected. According to the results of this study, application of the GAP scoring systems is recorded that mortality statistics in the Yazd hospital with low-risk group and moderate-risk group were better than expected. This is due to the proper functioning of the Yazd EMS and hospital emergency.
