چکیده مقاله:
In this completely novel and applied study, for the first time, it was investigated identifying hazardous points in design of processes/Parts of a PVC-based medical sterile Suction Tube Manufacturing Factory with assessing and controlling risks in Design Phase. This processes included storage, polymeric extrusion, polymeric injection, packaging in cleanroom, sterilization with Ethylene Oxide (EO) gas, process water pollution control and pollution control in manufacturing environment. Also parts included different parts of safety design of structure and construction of factory. With for achievement to this this important aims, incident reporting risk matrix method, as a very effective engineering method, was applied. It was investigated the hazards, causes leading to hazards and their effects. Also, it was identified the occurrence possibilities and severity of consequences. Moreover, risk levels were obtained. It was found that 43% of the risks had probability level of possible . Also, 70% of the risks had severity levels of major . According to results, 43% of the risks had high level as risk importance level. We proposed and designed prevention and control actions in designed phase according to concepts/principles of chemical engineering/ process engineering/ safety engineering/ environmental engineering completely and with mention of details.
