Comparison of the effects of Hugo’s point massage and play on IV-line access pain in children: A clinical trial study

Background and aims: Pain from invasive procedures such as Intra-Venous (IV) line access can cause physical and psychological stress in children hospitalized. One of the most important priorities in the nursing profession, especially in the pediatric ward, is to reduce pain. Based on this, the aim of this study was to compare the effect of Hugo s point massage and play on the severity of IV-line access pain in children hospitalized in the pediatric ward of Besat Educational and Medical Center in Sanandaj. Materials and Methods: In this clinical trial, 72 young children were selected by the Convenience Sampling method and assigned to three groups, i.e., control groups (24 subjects), play therapy (24 subjects) and Hugo point massage (24 subjects) by Random Allocation method. In the children of the massage group, 3 minutes before IV-line access until its completion, the middle angle between the first and second bones of the palm of the opposite hand was massaged by the researcher’s thumb on a rotating basis for one minute. Also, 3 minutes before IV-line access until its completion, the researchers encouraged the children of the play therapy group for bubble making play (by the gun bubble maker). A child demographic and clinical information questionnaire, a stopwatch device, and a Wong & Baker scale were used to collect information. Spss software version 22 and descriptive statistics and independent and paired t-test were used to analyze the data. Results: Independent t-test analysis did not show a statistically significant difference between the mean IV-line access pain in Hugo s point massage group and the control group. However, there was a significant difference between the mean IV-line access pain in the two groups of play therapy and the control group (p = 0.003 p <0.05). There was also a statistically significant difference between the mean IV-line access pain in both play therapy groups and Hugo s point massage group (p = 0.011 p <0.05). Finally, the analysis of variance test showed that there was a statistically significant difference between the mean IV-line access pain in the three groups of play therapy and Hugo s point massage group and control group (p = 0.003 p> 0.05). Conclusion: The results of the study showed a greater effect of play therapy on reducing IV-line access pain in young children. Therefore, it is recommended that play therapy should be considered as a low-cost and cost-effective way to reduce IV-line access pain in children.
