Background: Wound healing is one of the worrying issues in diabetic patients. Negative pressure ulcer treatment is one of the auxiliary methods that accelerates wound healing in these patients. The patient is a 63-year-old man with a known case of diabetic nephropathy. He has a history of type 2 diabetes from about 23 years ago. He has a history of hypertension and kidney disease for 3 years ago and has been treated with hemodialysis three times a week. The patient referred to Ali bin Abi Talib Hospital in Zahedan with a wound in the fifth finger of his left foot caused by trauma about two months ago. There was also positive evidence of osteomyelitis. The amputation of the fifth finger to the fifth metatarsal head of the left foot was performed. After surgery, the patient was washed with normal saline dressing but no improvement was achieved. Vacuum therapy was consulted for the patientMethods: The patient had a lesion on the left foot of a small finger with dimensions of 3×10 cm and depth of 1 cm. According to Wagner s classification, the grade of wounds was 3 and 4. According to the patient s advice, he underwent 3 sessions of vacuum therapy.Each session lasted 4 days and was then used to complete a new dressingResults: The wound was completely closed after 38 days of wound filling with granulation tissue and epithelialization.Conclusion: The patient was followed up for 3 weeks after discontinuation of treatment and no recurrence was observed.
