Medications related to dental implant failure: an updated systematic review

چکیده مقاله:

Background and Objective: Dental implants have been a successful option for replacement of lost dentition for nearly five decades, however failure in dental implants can occur. The possible risk factors have been investigated in recent years, such as systemic medications. The aim of this study is to systematically review the relationship between systemic medications use, and dental implant failure.Search Method: In this systematic review, based on PRISMA guidelines, two researchers independently searched MeSH and other relevant terms like dental implants , medications , failure in databases including Medline via PubMed, and Cochrane library; and by hand searching articles published up to August 2019 . grey literature available at the New York Academy of Medicine Grey Literature Report (http://greylit.org) and the register of clinical studies hosted by the US National Institutes of Health (www.clinicaltrials.gov) was searched to further identify potential candidates for inclusion; all observational studies matching our inclusion criteria were eligible to enter our assessment. Quality of the studies was assessed by Cochrane Critical Appraisal tools. Retrospective cohort, case-control and cross-sectional studies, that had been published within mentioned timeframe, met our inclusion criteria and those not focusing on implant failure , and other failure risk factors been excluded from the study.Finding: 922 articles were obtained by primary searching and after removing duplicate articles, 594 articles entered to our eligibility criteria assessment. 569 articles were excluded due to our exclusion criteria 2 articles didn’t have enough quality to be in our study and finally 23 studies reached the final stage of the study, including 5 studies focused on evaluating the association of implant failure and non-steroidal anti-inflammatory drugs (NSAIDs), 6 on selective serotonin reuptake inhibitors (SSRIs), 3 on proton pump inhibitors (PPIs), 9 on bisphosphonates (BPs), and one on anti-hypertensive (AHTNs). The qualitative analysis showed a significant relationship between PPIs and SSRIs with an increased implant failure rate.Conclusion: Although significant relationship between SSRIs and PPIs was obtained in this review, further clinical trial studies should be conduct to improve the quality of evidence. Also educational programs should be organized for clinicians considering implant therapy to be aware of possible medication-related implant failures.
