Lacto-N-fucopentaose (LNFP III) loaded electrospun Polydioxanone (PDO) scaffolds in treatment of non-healing wounds: a combination of biological and mechanical modulation of type 2 immune response and MQ polarization toward M2 macrophage

Introduction: Wound healing process is a well-known biological events in which various types of cells and cytokines are involved. In some skin damages such as non-healing wounds, medical interventions must be exerted to accelerate the healing process and avoid post-injury infections. Therefore, tissue engineering presents different strategies using various biomaterials to decrease the healing time and restore function of the damaged site.Type 2 immune response activation can be one of these strategies. Th2 immunity by recruiting the cells, which produce Th2-related cytokines, provokes re-epithelialization, angiogenesis and wound contraction. M2 macrophages, as a main cells involved in type 2 immunity, actively contribute to wound contraction, ECM deposition, and angiogenesis and fibroblasts activation and ultimately accelerate wound healing via secretion of different factors such as VEGF, IGF, ARG1, TGFβ, and MMPs. The role of mechanical forces in polarization of naïve macrophages has been well confirmed in literature. Thus, development of a scaffold with certain topological properties results in polarization of naïve macrophages into M2 macrophages.Conclusion: biological induction of type 2 immune response along with mechanical polarization of M2 macrophage can be an effective strategy in treatment of non-healing wounds. Here, we hypothesized that loading the milk derived oligosaccharide LNFP III, as an inducer of type 2 immunity, into an electrospun Polydioxanone (PDO) scaffold with 1 µm diameter has the ability tp accelerate the healing process of non-healing wounds.
