Soft tissue changes in class I borderline patients due to fixed orthodontic treatment with and without premolars extraction

Background and Objective: Space creation method is a decision concern in orthodontic treatment plan. Sometimes extraction of one tooth or more is necessary .The decision whether to extract or not, depends on the esthetic issues and stability of the teeth. The aim of this study is to evaluate and compare the effect of extraction and non-extraction treatment on soft tissue profile of borderline patients.Materials and Methods: In this retrospective study, cephalometric changes during orthodontic treatment for non-extraction and 4-premolar extraction patients were assessed. 70 Patients were chosen from patients referring to orthodontic department of dental faculty and clinic of Hamedan University of Medical Sciences. The following inclusion criteria were designed: skeletal Class I, 4-10 mm of space deficiency, and the possibility of both extraction and non-extraction treatments. All patients were treated with 22- slot edgewise fixedorthodontics and had all diagnostic records. Patients with congenital missing and large toothsize discrepancies, craniofacial anomalies, history of orthodontic or orthognathic surgery wereexcluded. Cephalometric analysis was done in two parts: hard and soft tissue.Finding: 35 patients in non-extraction group (11 males, 24 females with mean age of 17.46) and 35 patients in extraction group (9 males, 26 females with mean age of 18.46) were assessed. A significant difference was seen between extraction and non-extraction treatment in upper(-7.63±9.02) and lower incisors(-7.05±6.79), (p<0.001) and lips(lower lip: -1.56±1.97 and upper lip: -1.42±2.08), (p<0.001), both moving backward in extraction group. Upper(2.83±8.03)and lower incisors(4.63±5.47),(p<0.001) and lower lip (0.87±1.39, p<0.001)moved forward in non-extraction treatment. Mentolabial sulcus became shallower in non-extraction group(1.91±2.73, p<0.001) and more pronounced in extraction group(-1.89±4.2, p<0.001).Conclusion: Orthodontic treatment can change soft tissue appearance of lower third of face. Lips and incisors moved forward in non-extraction group and backward in the extraction group
