Background and Aim : Drug addiction is one of the most prevalent deviations of the present age, which infects innumerable victims every year and introduces new harm to family and community. Social workers learn and apply specific theories and skills in the field of social therapy in each of the social worker methods. The role of nurses and caregivers in preventing the return to women s addiction is also examined. Regarding functional theories, or changing and modifying the individual worker s attitude, the group s training in teamwork and the role of awareness and facilitation in community work can be considered as the most important and main task of a social worker. When one becomes aware of his own wrong behaviors and inappropriate ways of controlling the environment, he will be more prepared to learn alternative ways and behaviors.Methods : This is a descriptive cross-sectional study. Sampling was done in accessible form and the samples were made up to 200 men who had at least one history of returning to drug use. Data was collected face-to-face using structured interviews. Answers about the severity of the relationship between the relapse-related factors were presented at all, to a moderate, moderate, and high levelResults : : The results showed that 33.5% of patients with substance abuse once, 38% two to three times and 28.5% had more than three times unsuccessful treatment. 53% of them had been drug users in less than 3 months. Only 12% of them were able to remain drug free for more than a year. The mean of drug avoidance time was 6.3 ± 3 months. Based on the average scores, insomnia and temptation, the most important personal factors, and easy access to drugs, family conflicts and non-compliance were the most important environmental factors of relapse. Conclusion : Since psychosocial factors are the most important factors in the absence of cracks in the study, with a mean of 1.60.5, it seems that detoxification is not sufficient for the rest of the cracks to continue, and there are factors that are responsible for the return. It is suggested that further treatment centers should place mental health at the top of the treatment agenda, and strengthening the skills of coping with relapse in drug users is necessary
