PREGNANCY OUTCOME IN HIV POSITIVE WOMEN WITH ADDICTION: RESULT FROM A RETROSPECTIVE COHORT STUDY

چکیده
Background and Aim : in Iran HIV prevalence rate in women aged 15 to 49 was 0.1% in 2016. This rate appears to be increasing annually. Women living with HIV are potentially at increased risk of adverse pregnancy outcomes and Substance abuse significantly rise this risk. However, limited information exists on its impact on maternal and neonatal outcome. Our objective is to examine prevalence of substance abuse and pregnancy outcomes among HIV-Positive women.Methods : a retrospective cohort study was carried out in the hospitals affiliated to Mashhad University of Medical Sciences. Maternal and neonatal as well as pregnancy outcomes was assesses by mother’s HIV status. Multivariate logistic regression was used to estimate the adjusted effect of HIV status on pregnancy outcomes.Results : a total of 11 HIV-infected pregnant women and 96 HIV-uninfected women were enrolled in this study. substance use was reported by 90% of HIV-Positive women’s; the most common substance reported were opium (45.5%), crack (18.2%) and hashish (18.2%). Upon adjustment for baseline characteristics and substance abuse, variables remaining in the final model including: cesarean section (OR 11.8, 95% CI 1.45–95.9), abortion (OR 36.4, 95% CI 0.46–6.36), low birth weight of neonate (OR 0.96, 95% CI 0.19–4.84), congenital anomalies (OR 4.7, 95% CI 0.39–56.5), NICU admission (OR 0.18, 95% CI 0.39–0.91).Conclusion : Pregnancy in HIV-infected women how use substance is associated with adverse maternal and newborn complications. Illicit substance use was high in this groups. Monitoring of pregnancy outcomes in Iran will be important and Pregnant HIV-positive women should be followed in high-risk healthcare centers.کلیدواژه ها
