Introduction: Understanding the wide range of risk factors for Type 2 Diabetes Mellitus (T2DM) can facilitate diagnosis, proper classification and cost-effective management of the disease. This study is designed to identify the role of genetic and environmental (behavioral and biological) risk factors related to the development of diabetes and to estimate the progression of pre-diabetes and normo-glycemic condition to diabetes in third-generation of type 2 diabetic patients in Isfahan, Iran.Methods: This study is planned to assess the trend of diabetes in third-generation relatives of 13407 (40.5% male and 59.4% female) type 2 diabetic patients between the age of 35 years and older who were registered in the Isfahan Endocrine and Metabolism Research Center (IEMRC) from 1992 till now; in line with Isfahan Diabetes Cohort Study (IDCS), initiated in 2003, which is an ongoing cohort on 3156 (851 men and 2303 women) first-degree relatives (FDR) of type 2 diabetic patients. This cohort study is conducted from April 2018 and will be continued to assay the role of genetic and environmental (behavioral and biological) risk factors related to the development of diabetes in third-generation of type 2 diabetic patients in Isfahan, Iran.Conclusion: The results of this study will facilitate efforts to identify underlying genetic contributions to these complex phenotypes, since many traits such as general obesity, inactivity, body mass index, generally lifestyle change markedly with age.
