Methods: This hospital-based case-control study with 611 participants included 316 patient with myocardial infarction and 295 controls, matched by sex and age. All cases were MI positive according to electrocardiography and troponin test. Controls were negative for troponin test with no history of MI. Level of enzymes has been tested by Erba biochemical analyzer XL-600 and AUDIT AST, ALT and ALP product. This study was performed in Mazandaran Hear Center in three months period in 2018. For determining the Odds Ratio (OR), we used regression logistic analysis with 95% confidence intervals. The Statistical Package for the Social Sciences 16.0 (SPSS Inc., Chicago, IL, USA) was used for data analysis. P-value level was set at less than 0/05. Results: Of all participants, 64.3% of cases and 51.0% of controls were male with mean age of 61.28 (±13.8) in cases and 62.42 (±14.2) in controls. Average enzyme level in cases and controls were as follows; AST (cases= 168.5 IU/L, controls= 42.8 IU/L), ALT (cases= 82.2 IU/L, controls= 26.4 IU/L) and ALP (cases= 213.1 IU/L, controls= 218.6 IU/L). Regression logistic analysis showed no significant relationship between ALT and MI (adjusted OR= 0.7, 95% CI= 0.4-1.4, P-value=0.412) and ALP and MI (adjusted OR= 1.3, 95% CI= 0.5-1.3, P-value=0. 541. However, analysis indicated that the increase in the risk of MI is associated with AST level significantly (adjusted OR= 11.5, 95% CI= 6.78-19.5, P-value=0.000). Conclusion: Our study showed that high levels of AST could be associated with the risk of MI. Therefore, according to the results, clinicians may consider the AST level as the valuable diagnostic or monitoring factor for patients with MI to prevent the next attacks. Nevertheless, more comprehensive studies in different regions need to be performed to avoid the geographical and race biases, which may affect the matter.
