Intraosseous Spindle Cell Lipoma of the Anterior Maxilla: Review of Literatures with the Report of A Rare Case

Introduction: Oral lipoma constitutes 1% of all oral cavity tumors but it’s intraosseous and spindle cell variants are very rare. Here we report an intraosseous spindle cell lipoma that seems the first one in anterior alveolar ridge of the maxilla.Case: A 41-year-old man referred with a chief complaint of discomfort and swelling in anterior maxilla. The lesion has been developed since two years ago with an indolent growth rate. Facial asymmetry was obvious in clinical examination. Intraoral examination revealed a non-movable soft to bony hard asymmetric sessile mass in anterior maxillary alveolar ridge. Radiographic examination demonstrated a well-defined radiolucent intra bony lesion. CT imaging showed a hypo-attenuated mass with destruction and remodeling of cortical bone in the vicinity of left maxillary sinus anterior border in nasomaxillary junction area. Preoperative examination displayed no lymph node involvement or metastasis. Based on clinical and radiographic features, incisional biopsy was performed with primary possible diagnosis of calcifying odontogenic myxoma. A diagnosis of intraosseous spindle cell lipoma was made after histopathologic examination.Conclusion: Little is known about intraosseous lipoma specially oral spindle cell type, due to low number of oral intraosseous lipoma, rare cases of spindle cell type lipoma, and majority of asymptomatic lesions discovered accidentally. Therefore, proper attention to this entity is of crucial importance in diagnosis of jaw lesions.
