Cholecystitis, or gallbladder inflammation, is one of the most common diseases of the gastrointestinal tract, which may be acute, chronic or acute in chronic conditions. Given the high morbidity and mortality of this disease, early diagnosis of the disease with less invasive preclinical methods can reduce the adverse outcomes of the disease.Material and Methods: In this descriptive cross-sectional study, all patients underwent cholecystectomy, in Besat hospital of Hamadan, in the first six months of the year 1395 in terms of variables, demographic, solography, pathology and CBC diff were evaluated. The ratio of neutrophil to lymphocyte was calculated and its sensitivity and specificity were compared with pathologic results of the patients. SPSS software version 16 was used to analyze the data and the significance level was less than 0.05.Results:The total number of patients studied was 197, 22.8% male and 77.16% female, with mean age 45.39±17.77 years. Based on the pathological results of patients undergoing cholecystectomy, acute, chronic, gangrene and perforated cholecystitis were 38%, 34%, 16.8% and 11.2%, respectively. Sensitivity and specificity of NLR than pathology results to screening the acute cholecystitis (gangrene, perforated) from chronic cholecystitis with NLR=3 was 62.31% and 58.86% , NLR=4 was 33.85% and 94.03% and with NLR=5 was 23.07% and 94.33% respectively. There was no statistically significant difference between the acute and chronic cholecystitis, but in terms of age, the mean age of patients with acute cholecystitis was significantly higher than chronic cholecystitis (P <0.05). Conclusion: The ratio of neutrophil to lymphocyte in acute cholecystitis increases significantly. This ratio has the highest sensitivity at the cut point2 for the screening of acute cholecystitis (gangrene and perfection). By increasing the cutting point level, the sensitivity increases and specificity decreases
