Introduction: Regardless of acute revascularization, the fate of 30% of patients suffering ischemic heart disease will be heart failure (HF) within six years. So, can PEG play a protective role in minimizing heart failure Body: PEG has confirmed membrane-protective effects in a multiplicity of cells and organs against various injuries. The protective effects of PEG was seen in cardiac myocytes against hypoxia-reoxygenation (H-R) injury in vitro, previously. In addition, pretreatment of neonatal rat with PEG protected ventricular myocytes against (H-R) injury. Indeed, use of PEG declined cell death in cardiac myocytes in vitro was connected to a significant decrease caspase-3 activity and reactive oxygen species (ROS) production through protection of membrane integrity. The constructive properties of PEG in sarcolemma membrane integrity are by maintenance of lipid-raft domains. In addition, use of PEG compounds in combination with standard cardioplegia solution has been utilized for myocardial protection in the transplantation setting, experimentally and clinically. This can be explained through the inhibition of cell death and enhancement of cell survival signaling leading to protection of ventricular function. Conclusion:Taken together, use of PEG seems to be as a candidate to reduce the cell death that can lead to impaired function and heart failure.
