Iron overload and glucose metabolism in patients with B-thalassaemia major: systematic review

چکیده مقاله:

Thalassemia major (TM) is a type of anemia that characterized by a deficiency in the production of ß globin chains in resulting in ineffective erythropoiesis. Multiple blood transfusions are critical for survival in these patients. Iron overload is an uncommon complication of transfusion that occurs in patients after long periods that can precipitate in tissues and causes many problems. In patients with TM, uncontrolled iron overload has serious clinical consequences with considerable morbidity and mortalityMaterial and Methods: This study performed by using collected article in English were available on details of the main topic since 2005 to 2018 in Scopus, PubMed, google scholar, online library wiley and science direct. In total, 35 article were collected of all article found and keyword such as Diabetes mellitus, thalassemia major Iron overload were used by two researcher independetly to search in databases. Finally, the articles found on the basis of the exclusion criteria were based on the abstracts without the main text of the article, as well as texts that were not related to the topic, and based on the PRISMA model, 20 articles were included.Findings: Result of this study show that Diabetes Mellitus (DM) is one of the most common Endocrine abnormalities in this patient. Prevalence of DM ranges from 6.4 to 14.1 in crosssectional studies. Several test has been show that both insulin resistance and decreased insulin secretion contribute to DM in thalassemia major patients. Previous studies showed that DM correlate with derum ferritin concentration, hepatitis C infection, and ....Conclusion: In conclusion this review showed the onset of diseases in patients with TM can be silent. Therefore specific screening studies are needed. According to the result, early recognition and prevention of the endocrine complications, by early and regular chelation therapy, is mandatory for the improvement of the quality of life and psychological outcome of these patients.
