Change in Burn Pattern in a Referral Burn Center; Are Leisured Days Coming

Background: Burn injury is a common health problem especially in developing countries. Besides, the pattern and severity of burn injury are different from developed ones. Due to rapid changes in these countries, being unaware of transitions in burn patterns can cause an irrecoverable burden to the health system. The aim of this study was to evaluate an 8-year trend of burn characteristics in a tertiary hospital in the northeast of Iran.Methods: All admitted patients with acute burn during 2010-2017 were evaluated. Demographic and clinical information regarding admitted burn patients were gathered from the hospital information system. Any missing data was checked by patients’ medical records. Data were analyzed using SPSS 25.Results: Overall, 6098 patients were admitted during this 8-year period. Male predominance was observed in the whole study period (from 62.5% in 2010 to 63.5% in 2017, p=0.05). Mean age was also constant (25.2±18.7 years in 2010 to 26.6±21.3 years in 2017, p=0.265). However, burn mechanism was significantly changed: Fire (50.4% in 2010) was decreased to 34.9% in 2017 and scald was 21.15 in 2010 was increased to 36.0% in 2017, and contact (0.4% in 2010) was increased to 5.0% in 2017 (p<0.001). Burnt total body surface area (TBSA) was also changed: 0-10% burnt TBSA was 3.3% in 2010 and 27.4% in 2017, 11-20% burnt TBSA was 15.1% in 2010 and 28.8% in 2017 while 80-90% burnt TBSA was 5.6% in 2010 and 1.8% in 2017, > 90% burnt TBSA was 7.7% in 2010 and 3.1% in 2017 (p<0.001).Conclusion: Health care specialists and decision makers should be aware of transitions in patterns of burn in recent years. These are valuable information, especially for long-term health planning. However, a country-based survey is highly suggested to enlighten the condition of other major burn centers in this country.
