Comparison of early and late postpartum glucose tolerance test in patients with gestational diabetes mellitus

Objectives: To evaluate if postpartum diabetes mellitus screening could be done during post delivery hospitalization. Materials and methods: This case series study was performed in women with gestational diabetes who had normal blood sugar after delivery.130 women with GDM took 75 gr oral glucose tolerance test (OGTT) before hospital discharge and repeated the test 6 to 12 weeks later. Results: In this study 130 women were participated, 107(82.3%) patients were analyzed. Early OGTT, was abnormal in 46 cases (43.0%); prediabetes in 34 (31.8%), diabetes in 12 (11.2%). 6 to 12 weeks later OGTT was abnormal in 28 cases (25.2%); 22 (20.6%) prediabetes and 6 (5.6%) diabetes. In 5 patients (4.7%) with normal early OGTT, 6 to 12 weeks later OGTT was abnormal (prediabetes and diabetes). There was a significant association between results of early OGTT and 6 to 12 weeks later OGTT p=0.00. Early OGTT was able to detect prediabetes and diabetes (abnormal results) with a sensitivity of 82.14% (95% CI, 63.11-93.94), specificity of 70.89% (95% CI, 59.58-80.57). Conclusion: Early postpartum screening effectively diagnosed women who were at high risk of diabetes mellitus and needed to undergo OGTT 6 weeks later. Identifying high risk patients who need to return 6 to 12 weeks later, eases the management of patients and sensitizes patients to come back to check.
