Colorectal cancer or (CRC) is the third most common cancer . its Survival rates is poor due to delays in early stage detection . One major drawback forward to current Diagnostic strategy is colorectal cancer heterogeneous nature unique to an individual’s tumor. By reviewing previous strategy in cancer treatment, it can be understood that the best strategy is personalized treatment due to individual-based laboratory finding. Thus far, a number of studies attempted to evaluate the potential of Cell derived extracellular vesicles (CDEVs) or exosomes as appealing particles biomarker to early detection of CRC. Membrane vesicles secreted by a variety of mammalian cell such as intestinal Mesenchymal stem cell. A wide range of functions for Exosomes, reveled to be paracrine .they can transport and deliver various amount of proteins (involved in cell structure, cell adhesion, and signal transduction) , lipids ,microRNAs, and mRNAs which vary according to the source that secreted them. Exosomes surface antigens such histocompatibility complex (MHC) class I and II molecules have provided deeper insights about Exosomes as an novel therapeutic approaches and appealing candidates to personalized treatment and diagnose. The purpose of this paper is to review recent research about the exosomes focusing on their potential as therapeutic approach. Then, we describe the various challenge which are face to this era of cancer investigation.
