Over the past two decades, the view about adipose tissue dramatically changed from an inert energy storage source to an active endocrine organ. Communication of adipose tissue with other central and peripheral systems are through synthesis and release of a host of molecules that we generally refer to as adipokines. The levels of some adipokines correlate with specific metabolic states and has direct impact on the metabolic homeostasis of the system. Epicardial adipose tissue (EAT) is now recognized as a metabolically active organ which might significantly affect cardiac function and coronary artery response through the production and release of various bioactive molecules, such as adiponectin, resistin and inflammatory cytokines. Substantial progress in molecular cardiology, along with an enthusiasm for personalized medicine, has led to a rapid expansion in the field of adipokines and biomarkers in recent years. Metabolomics is one of the new omics which enable to create f a photograph of the metabolic state of an individual exposed to different pathologies and environmental factors. Nowadays novel and specific analytical methods are required to understand the metabolic shifts that occur even before the pathological state to find possible early predictive biomarkers and to provide a faster diagnosis and more personalized treatment. EAT may be a future target not only for discovery of biomarkers but also for the pharmacologic intervention.
