Ocular toxocariasis: four series of cases

چکیده

IntroductionToxocariasis, an infection caused by nematode parasite, is found globally but the majority of cases occur where dogs and cats are kept in close proximity to human. This infection can be symptomatic (ocular, visceral, and neurotoxocariasis) or asymptomatic (covert toxocariasis). Ocular form of toxocariasis is rare. Cases reportEight-cases suspected for ocular larva migrans with eye complications were admitted to our department. After running ELISA tests for anti-Toxocara antibody, four of them were reported positive. The first one, 25-year-old female (student) presented with chronic granulomatous posterior uveitis, photophobia, chorioretinitis, blurred vision, posterior pole granular and past history of cat contact. The patient was referred to the lab for Toxocara IgG checking. After two times the test, result was positive. Eosinophil rate was reported 1.6%. Second case was 30-year-old male (carpet seller) with acute non-granulomatous posterior and intermediate uveitis, chorioretinitis, blurred vision and no history of animal contact. Eosinophil rate was 2.3% and serological test for Toxocara-IgG was positive.Third, 47-year-old house wife woman with no animal contact and presentation of chronic non-granulomatous pan-uveitis, retinal inflammation, blurred vision and cataract. Hematology exam with 0.7% eosinophil and ELISA method was positive for antibodies against Toxocara.Finally, in the fourth case 52-year-old female (farmer) reporting of cat contact with manifestations of subacute granulomatous posterior uveitis, blurred vision and eye pain. Strongly serological positive reaction with 0.7% was reported.All these four cases were diagnosed with ocular toxocariasis and patients underwent the related treatment. Discussion and ConclusionThe diagnosis of ocular toxocariasis can be challenging, both because the condition is relatively uncommon and its presentation varies from patient to patient. Diagnosis of ocular toxocariasis is based on clinical findings and Laboratory techniques (ELISA WB). There are lot of differential diagnoses like retinoblastoma, so correct and quick diagnosis and treatment is very important.
