The prevalence of obesity in dialysis patients is increasing, and as a result, more obese dialysis patients are being a candidate for kidney transplantation. Many transplant centers are using BMI of 30 or 35 kg/m² as exclusion criteria for kidney transplantation. In this review, we describe an approach for applying available data on the importance of obesity in kidney transplant recipients, the risk of obesity-related surgical complications and interventions to optimize the health of this population.Methods: We conduct a comprehensive search in Medline, PubMed, Up-to-date and Cochrane databases.Results: Several studies were shown that obese patients have more complications, shorter transplant, delayed graft function and shorter patient survival than ideal-weight patients. Documents and Data for obesity and post-transplantation complications are controversial. There are few clinical studies on managing obese patients with end-stage renal disease (ESRD) and no acceptable strategies for different problems exist in this population.Conclusion: Suggestion of lifestyle modification, physical activity, and calorie restriction are the first approach for these patients. Pharmacological treatments are generally unsafe in these patients, except orlistat, but with mild efficacy. Bariatric surgery may be the only option for morbid obesity if all other treatments fail. Intervention should emphasize behavior therapy, physical activity to increase muscle mass, and calorie but not protein restriction. Bariatric surgery should be considered in severely obese patients who are kidney transplant candidate
