EFFECTS OF SYMBIOTIC SUPPLEMENTATION IN ADJUNCT WITH NONSURGICAL-PERIODONTAL-TREATMENT ON METABOLIC AND PERIODONTAL STATUS IN TYPE-2 DIABETIC PATIENTS WITH PERIODONTAL DISEASE

Background and Aim: The aims of this study was to determine the effects of symbiotic supplements in adjunct with nonsurgical periodontal treatment on the glycemic control and periodontal status, inflammatory mediators and lipid profile in type 2 diabetic patients with periodontal disease.Methods: In this randomized double-blind placebo controlled clinical trial, 47 patients suffering from DM and periodontal disease were recruited and randomly assigned to two groups: intervention group (n= 23), received one capsule of multispecies probiotic plus 100 mg fructo-oligosaccharide supplement (500 mg in each capsule) every day and control group (n=24) received placebo capsule containing 500 mg wheat flour for 8 weeks. All patients were treated with non-surgical periodontal treatment during the intervention period. A 3-day 24-hours food recall, physical activity, anthropometric parameters, hemoglobin A1C (HbA1C), fasting blood sugar (FBS), Tumor necrosis factor alpha (TNF-α), Interleukin 6 (IL-6), high-sensitivity C-reactive protein (hs-CRP), lipid profile and pocket depth (PD), were measured before and after the intervention.Results: Probiotic supplement decreased serum levels of FBS, HbA1C, TNF-α, IL-6 and PD, compared with the baseline values. In addition, a significant difference in PD was observed between two groups after intervention. Furthermore LDL-C levels significantly decreased compared with the baseline value in both groups. No significant changes were observed in total cholesterol, triglyceride and HDL-C within and between groups.Conclusion: Probiotic supplementation as adjuvant therapy with nonsurgical periodontal therapy may be beneficial in improving glycemic control, inflammation and periodontal health and decreasing LDL-C in patients with diabetes with periodontal disease.
