Background and Aim: This double blinded randomized clinical trial was aimed to find the effect of vitamin D supplementation on diabetes control factors including fasting serum glucose and insulin, HbA1c, and insulins resistance and sensitivity, Irisin and SIRT1 among type 2 diabetic patients living in Bandar-E Anzali, North Iran. Methods: We recruited 90 diabetic males and females into placebo and intervention group with mean ± SD age of 50.05±10.17 and 50.36 ±10.2 respectively (p=0.89), and Body Mass Index (BMI) ±SD of 31.37 ±3.4 and 30.43 ±3.2 respectively. The participants were diabetic vitamin D deficient with HbA1c serum level<8.5% and we excluded those who were using vitamin and mineral supplements, or had any acute disease, or recently modifies dose or type of their hyperglycemia medications. The intervention group took 50000 IU vitamin D supplement weekly for 8 weeks. Independent t-test and paired t-test were used to compare the mean outcomes between two study groups as well as the comparison between baseline and after intervention. The probable confounders were used via ANCOVA Regression models (significant p-value<0.05).Results: Serum vitamin D, SIRT1 and Irisin increased significantly among intervention group (p<0.001). HbA1c decreased significantly by 1%. Fasting glucose and insulin remained unchanged in both groups. Conclusion: Vitamin D supplementation may help controlling type 2 diabetes by decreasing HbA1c and increasing SIRT1 and Irisin levels among vitamin D deficient diabetic patients.
